IFG LifeCHOICE is an insurance product of PT Asuransi Jiwa IFG Life, which is licensed and supervised by the Financial Services Authority. Therefore, IFG LifeCHOICE product is NOT a Bank BTN product and Bank BTN is not responsible for the policy issued by PT Asuransi Jiwa IFG.

Insurance Benefits
- Non-accidental Death
100% of the Sum Assured if the death is not due to an accident. - Accidental Death
Compensation of up to 200% of the Sum Insured if the death is caused by an accident within 90 days after the accident occurred. - Critical Illness
Critical illness benefits if the Insured suffers or is diagnosed with one of the critical illnesses as listed in the List of Critical Illnesses/Conditions for three stages (early, intermediate, and late), with benefit payments between 50%-100% of the sum insured, according to the severity of the condition. - Premium Return Benefit
Premium returns up to 106% if there is no benefit claim or death during the active policy until the end of the insurance period.

*The Premium Return Benefit value may be adjusted periodically and the applicable benefit value is stated in the Policy, as well as applies from the purchase of the Policy until the Insurance Period of the Policy ends.

Conditions

Premium Payment Requirement
- Prospective Policyholders are required to pay the Premium to the Insurer according to the Premium amount and Premium payment method as stated in the Policy
- Premium payment is considered valid, if the funds have been received fully by the Insurer

Claim Submission
- Prepare claim documents, fill out completely, and sign the claim submission form
- Download the claim submission form from IFG Life's official website/media
- Submit all claim documents according to the claim submission requirements to the Insurer's office or directly visit the nearest Walk-in Customer & Insurer Representative Office or BTN Branch Office
- The Insurer receives, examines, and analyzes the submitted claim documents. To find out the claim's status, the Policyholder/Insured can contact the Call Center 1500 176 or its revised number
- If the claim is approved, the Insurer will pay the claim payment no later than 14 (fourteen) working days after the claim decision is approved.

These are the conditions when Mrs. A was affected by Critical Illness and still alive during the Insurance Period.
- Mrs. A is diagnosed with one of the Critical Illnesses, namely Early Stage Cancer. After the waiting period and passing the survival period, the Early Stage Critical Illness benefit will be paid in the amount of IDR 84,000,000.
- If Mrs. A is diagnosed with one of the Late Stage Critical Illnesses and the Early Stage Critical Illness benefit has previously been paid to Mrs. A, the remaining Sum Assured will be paid minus the Critical Illness benefit in the Early Stage Critical Illness condition, which is IDR 84,000,000.
- If Mrs. A is still alive until the insurance period but has submitted a Critical Illness Benefit claim, she will not get the return of premium benefit at the end of the insurance period.

These are the conditions when Mrs. A was affected by Critical Illness and then Death Not Due to Accident.
- Mrs. A is diagnosed with late-stage cancer and has never previously submitted an Early Stage Critical Illness claim. After the waiting period and passing the Survival Period, a Late Stage Critical Illness benefit of IDR 168,000,000 will be paid.
- In the 6th Policy year, if Mrs. A dies due to the disease suffered, the Accidental Death benefit will be paid for IDR 168,000,000. After the Accidental Death benefit is paid, the Policy will be terminated.
- Mrs. A does not get the Return of Premium Benefit at the end of the Insurance Period because the Policy ends when the Insured dies.

- Mrs. A is diagnosed with one of the Critical Illnesses, namely Late Stage Cancer, and has never previously submitted an Early Stage Critical Illness claim. After the waiting period and passing the Survival Period, a Late Stage Critical Illness benefit of IDR 168,000,000 will be paid.
- In the 6th Policy year, if Mrs. A dies due to an accident, the Accidental Death Benefit will be paid with a total Sum Assured of IDR 336,000,000.
- Mrs. A does not get the Return of Premium Benefit at the end of the Insurance Period because the Policy ends when the Insured dies.
